Risk factors for thromboembolic events in children and young adults

Abstract

Background: Thromboembolic events (TEE) in children are rare. They develop through concomitant hemodynamic disturbances in the cardiovascular system or as a consequence of hereditary disorders of the haemostatic system. When dealing with children, suffering from TEE, both possible primarily inherited conditions and acquired prothrombotic states as well as exogenous risk factors should be investigated. Patients and methods: In our research we analyzed medical records of 86 children and young adults who either suffered from TEE or were referred to our outpatient clinic because of positive family history for thrombophilia and at least one thrombophilic risk factor. TEE occured in 50 cases. We were able to detect thrombophilic risk factor in 70 %,  the most frequent being homozygous mutation in the MTHFR. In 28 % children with TEE, we specified the triggering factor, oral contraceptive therapy as the most common  in one third of cases. Conclusions: In order to make a proper decision on the initiation and duration of the secondary antithrombotic prophylaxis and the need for the initiation of the primary prophylactic measures, it is very important to determine inherited trombophilic risk factors.
