Vitrektomija pars plana pri zapletih operacije sive mrene

Abstract

Izhodišča: Z razvojem vitreoretinalne dejavnosti in poznavanjem patologije se je napoved izida po nekaterih zapletih operacije sive mrene izboljšala. S pomočjo operativnega posega vitrektomija pars plana lahko preprečimo poslabšanje vidne funkcije pri nekaterih med- in pooperativnih zapletih operacije sive mrene. V primeru zapletov operacije katarakte je pomembno njihovo čim zgodnejše diagnosticiranje ter ukrepanje. Namen članka je prikazati zaplete operacije sive mrene, ki jih rešujemo s pomočjo vitrektomije.

Zaključki: S preventivnim ukrepanjem pri bolnikih z večjim tveganjem za ekspulzivno krvavitev lahko zmanjšamo verjetnost, da nastopi krvavitev. S pravilnim ukrepanjem pa zmanjšamo poslabšanje vidne funkcije, če ekspulzivna krvavitev nastane. Pooperativni endoftalmitis je urgentno stanje, ki zahteva takojšnje ukrepanje. Edino ustrezno zdravljenje je intraokularno dajanje antibiotika. Pri potopljenih lečnih masah je smiselna zgodnja vitrektomija, saj se tako izognemo vnetju v očesu, dosežemo hitrejšo rehabilitacijo in boljšo pooperativno vidno ostrino.
